What to Bring To Try-Outs
**A check for $10 made payable to the Churchville Youth
Association (insurance fee). This check will not be returned to the
athlete if they do not make the team.
**A copy of the athlete’s Birth Certificate (if you are new to CYA).
**A completed CYA Registration Form (click for form).
**A check for $245 made payable to the Churchville Youth
Association (summer soccer registration fee). This check will be
returned to the athlete if they do not make the team.
**Soccer cleats, soccer ball, shin guards, water bottle.
