- A copy of the athlete’s Birth Certificate (if you are new to CYA).
- A completed and signed CYA Registration Form (click for form).
- If athlete makes the team, Registration Fee is $265.00 made payable to the
CYA Soccer. (due October 15th, 2011)
- Soccer cleats, soccer ball, shin guards, water bottle.
